Immunization Requirements for College Students
Required Vaccination Information
If you plan to register for an in-person course, participate in on-campus activities, or live at The Suites at Laker Landing, you must submit the medical information listed below. Check with your physician or high school to obtain official immunization records.
Measles, Mumps, Rubella - Proof of Immunity
These are typically combined into a vaccine called MMR. Proof of 2 MMR vaccinations satisfies this requirement. Serological evidence of immunity (titers) to measles, mumps, and rubella are only accepted with certified lab results. Proof of honorable discharge from the armed services within ten years of your college application also meets the NYS requirement for MMR and Meningitis.
New York State Public Health Law 2165 requires students born on or after January 1, 1957, attending college within New York State to show proof of immunity* against measles, mumps, and rubella. Persons born before January 1, 1957 are exempt.
Proof of immunity must be provided in order to register at Finger Lakes Community College. Law, and College policy, will not permit continued attendance by individuals failing to present the required proof by the 30th day of the semester, unless the student presents: (i) a certificate from a licensed physician or nurse practitioner certifying that such immunization may be detrimental to the Student’s health or is otherwise medically contraindicated (in which case the requirements may be waived until such immunization is determined no longer to be detrimental to the Student’s health or otherwise medically contraindicated); (ii) proof of honorable discharge from the armed services within ten years from the date of application to Finger Lakes Community College (which shall qualify the student to attend the College pending the actual receipt of immunization records from the armed forces); or (iii) a written and signed statement from the student, or parent or guardian, if applicable, that s/he holds genuine and sincere religious beliefs that prohibit immunization of the student.
Proof of Immunity:
MEASLES: Two doses of live measles vaccine (given 1968 or after). The first dose must have been given on or after 12 months of age. The second dose must have been given on or after 15 months of age.
MUMPS: One dose of live mumps vaccine received on or after the first birthday.
RUBELLA: One dose of live rubella vaccine received on or after the first birthday.
Meningococcal Meningitis Response Form — this is typically given as a series of vaccinations (MenACWY and MenB) starting when kids are 11 or 12 years old. All students (except Gemini) must either submit meningitis vaccination documentation or sign a form acknowledging that you understand the risks and choose not obtain immunization against meningococcal/meningitis disease.
Exemption for Online Students — If you are a fully online student, you are exempt from the above immunization requirements. Hybrid or part-time students who take at least one course on campus must still meet immunization requirements.
Exemptions - Students may apply for a medical or religious exemption to all vaccinations, or for only COVID-19 vaccination. Exemptions will be considered after initial registration and prior to the start of any term.
Withdrawal - Students who have not submitted proof of immunity will not be allowed to register for in-person coursework. Students who do not have proof of immunity may be given a registration deferral through Student Health. This deferral will be good for no more than 30 calendar days. At that time, if proof of immunity is not received, the student may be administratively withdrawn from all in-person classes. Students administratively withdrawn from classes, due to their failure to comply, will not be granted refunds of tuition or fees.
Outbreak - In the event that an outbreak of one, or more, of the illnesses noted above occurs, access to College facilities will be restricted only to those having proof of immunity. Students who are not fully immunized, or who have exemptions, will not be allowed access to the campus and are still responsible for all coursework. If the student is unable to complete coursework remotely, s/he/they will still be financially responsible.
Recommended Vaccines
In addition to the required vaccination information above, FLCC health professionals also recommend the following vaccinations:
- COVID-19 — safe and effective. Annual COVID-19 vaccines are recommended in the U.S. Ask your primary care provider for specific vaccine recommendations.
- Influenza — also known as flu shots. Recommended for everyone 6 months and older as soon as possible each flu season.
- Mpox — a 2-dose vaccine that protects against mpox and smallpox infections.
How to Submit Your Immunization Information
Please submit the appropriate immunization forms promptly. Students who have not submitted proof of immunity by the 30th day of the semester will be administratively withdrawn from classes. Immunization information can be submitted online through your Student Health Portal by following these steps:
- Log in to the Student Health Portal through your MyFLCC page
- Select "Document Upload"
- Choose document type "Immunizations"
- Take a photo of your immunization record, vaccination card, or other documentation
- Upload and submit your documentation
You can also download and print our immunization form located on the Center for Student Well-Being webpage. Once completed by your healthcare provider, the form can be mailed to Student Health Services, 3325 Marvin Sands Dr., Canandaigua, NY 14424 or dropped off at the Center for Student Well-Being (Room 3260).
